How it works on the unit

Each document condenses to a short file note: what changed, what is requested of the practice, what needs follow-up.
A clinician or nurse confirms the summary against the source before it is accepted to the record. The source document stays attached.
Requested actions, results to chase, review timeframes, are listed rather than buried on page three.
